First Visit Information

patient infoFor your first appointment at Palmetto Oral & Maxillofacial Surgeons, P.A. you will meet with one of our surgeons to discuss the reason for your visit. The surgeon will review your medical history, perform any imaging studies (x-ray, computer modeling, photography), and determine a diagnosis. Additionally, for complex procedures, we may consult with your referring doctor and create digital 3-dimensional imaging. Once the surgeon has determined a treatment plan, he will discuss it with you and answer any questions you may have.

A preoperative consultation is mandatory for all patients. Please inform our medical staff if you have any medical conditions prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) and all of your current medications (i.e., heart medications, aspirin, anticoagulant therapy, osteoporosis etc.).

To assist us during your consultation, please review and follow the instructions on our Pre-Surgical Instructions and bring the following documentation:

  • Your referral forms and any x-rays
  • Completed Patient Registration Forms and Medical History
  • A list of current medications
  • Medical and/or dental insurance information.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian during the consultation visit.

Financial Policies

Palmetto Oral & Maxillofacial Surgeons strives to provide the highest quality medical care at a reasonable cost to you. You will be required to pay a deposit on the morning of the procedure based on your insurance. We file all insurance and refund any overpayments to you at the earliest opportunity. Your medical and dental insurance cards are REQUIRED to file your insurance.

For your convenience, we accept Visa, MasterCard, and Discover. 

You are responsible for all expenses not covered by your insurance or deposit. Fees for surgical procedures are an ESTIMATE only; fees can be reduced or increased based on the complexity of the procedure(s) and related services.

You will receive a statement each month detailing your balance and fees paid. Most insurance companies will respond within four to six weeks. Please contact our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
